BASIC LIFE AND AD&D INSURANCE provided by the Standard 

All full-time employees are provided with 100% of your annual earnings in multiples of $1,000 to a maximum benefit of $150,000 of basic group life/AD&D insurance. The City of Peachtree City pays the full cost of this life and AD&D insurance. Please contact Human Resources to update your beneficiary. This benefit reduces by 65% at age 69 and 50% at age 70.

OPTIONAL EMPLOYEE LIFE INSURANCE provided by the Standard

Employees who want to supplement thfamilyeir basic life insurance benefits may purchase additional Term Life coverage for themselves and their dependents. When you enroll yourself and/or your dependents in this benefit, you pay the full cost through convenient payroll deductions.

• Benefit Amount = Increments of $10,000 (minimum $10,000) 

• Guaranteed Issue = $200,000* 

• Evidence of Insurability required for all late entrants 

• Maximum Benefit = $500,000 

• Benefit Reduces by 65% at age 69 and 50% at age 70

OPTIONAL SPOUSE BENEFIT provided by the Standard

Employees who want to supplement their basic life insurance benefits may purchase additional Term Life coverage for their dependents. When you enroll your spouse in this benefit, you pay the full cost through convenient payroll deductions. 

• Up to 50% of employee amount in $5,000 increments 

• Guarantee Issue = $25,000* 

• Maximum Benefit = $250,000 

• Benefit Reduces by 65% at age 69 and 50% at age 70

OPTIONAL CHILD BENEFIT provided by the Standard

• Child Benefit = $5,000 for $0.18 bi-weekly payroll deduction 

• To age 19; To age 24 if a full-time student 

*You may be eligible for an increase of two increments up to the Guaranteed Issue during Open Enrollment. Please refer to your Plan Document for additional details.

Important Terms to Understand:

EVIDENCE OF INSURABILITY: Evidence of insurability is a request to verify good health and is often in the form of a questionnaire. This is required when you are requesting insurance that is over the guaranteed issue amount or if you are enrolling after initial enrollment. 

GUARANTEED ISSUE: Guaranteed issue is the amount of life insurance that you can elect without having to provide evidence of insurability. The guaranteed issue period is 31 days from the date you first become eligible for the plan. If you choose not to enroll when you are first eligible and enroll at a later date, the entire amount of insurance will be subject to evidence of insurability.
